If it is the same medication that is wrong every time then I really think it is the Dr to blame.
The pharmacist can only issue what is on the prescription.

When you next see your Dr ask him to check on his computer (just about all surgeries are computerised now) and make sure that they have the correct details for repeat prescription for that item. I will bet you now that what is on their computer is incorrect! Make sure he/she changes it there and then.

I had allergic reaction to Amoxycillin in 1999 and every time Dr has gone to prescribe antibiotic they would start to issue a penicillin based one and every time I had to remind them of the allergy. Finally earlier this year the Dr actually flagged the allergy on the computer!

I take Warfarin in various combinations (1mg, 3mg and 5mg tablets) and for ages the prescription for the 1 mg said "take one twice a day" which was nonsense.
